Resilience DVD Usage Agreement

SAFEchild loans the Resilience DVD to community organizations interested in taking action to prevent Adverse Childhood Experiences (ACEs), to build resilience and to use trauma-informed approaches in Wake County.

Per the licensing agreement for our purchase of the film:

1) you may not charge admission for viewing.

2) your audience must be limited to fewer than 250 people.

Further, any organization borrowing the DVD bears the responsibility of:

1) picking up the DVD from SAFEchild’s office at 864 West Morgan Street, Raleigh, Monday-Thursday during business hours (9am-5pm).

2) returning the DVD within two business of the screening; and

3) completing this short online survey within one week of screening.

Any organization borrowing the DVD is responsible for any damage to the DVD. If the DVD is damaged and/or not returned within two business days, we will need to charge a $400 fee as replacement cost and will send an invoice.

Use of the DVD is contingent upon submission of the below form to Lisheema Barr, program coordinator for the ACEs Resilience Initative. We will honor requests on a first-come, first-served basis. Please contact Lisheema at lbarr@safechildnc.org with any questions.
